I have reflowed the pot solder joints (again) with no difference.

yes, the noise is present with the source connected, I am using a classic ipod connected via a 6" dock cable, and the noise is present with no source connected.

With all the info so far, there doesn't seem to be an obvious answer except perhaps you have a bad U5. It's not "dead", but one or more of its internal transistors or other component may have become noisy.
